Worker Registration
  1. That I authorize (Own Country Licensed Recruitment Agency; Training Center; Hong Kong Employment Agency: and related third-party partners this include (e.g. YouTube, Facebook, WhatsApp, Line, WeChat, Weibo, Twitter, Instagram, HelperDB, and other online social media) to submit my bio-data and personal information (e.g. name, address, photo, video, handwriting resume, HKID and/or passport numbers, age and DOB., religion, body measurements, fingerprint, etc.), their family members (e.g. name, age, occupation, etc.) stored and recorded in their database system to allow the public and/or future prospective employers to view my said bio-data and personal information for my employment purposes;
  2. That the I hereby certify the correctness and truthfulness of my said bio-data and personal information;
  3. That I have read and understood the contents hereof, and no fraud, force, violence, or intimidation has been used against me;
  4. That I have executed this Affidavit knowingly, willingly, and voluntarily to attest to this Affidavit and am fully aware of its consequences.


I hereby acknowledge that I have read, understand and agree to the above terms for my employment purposes.
